The timely and and accurate completion of a patients discharge summary, connects the care we provide back to the patients clinical and social support network.

What we like:
- The consistency in our performance in 2020.
- The improvement that occurred in 2020 when comparing our overall performance to 2019.
What we would like to see improve:
- Continued increase in compliance at Williamstown ED.
- Compliance rates above 80% at all three sites
