Why is this important:
Is the acronym created to help standardise clinical handover. Aiming to reduce failures in communications, while improving the quality & safety of patient information from staff to staff member. This formula should be used to complete handover at Western Health.

In using ISBAR staff will support safe, coordinated care, by:
- Standardising our approach to clinical handover
- Identifying the patient
- Explaining the patients situation and presentation
- Explaining the patients medical and social background
- Communicating any Alerts & Allergies
- Reviewing results & requests
- Detailing and communicating the current plan
- Documenting and communicating current lines & devices
- Including the handover sticker in the patients clinical notes
