Why is this important:

Is the acronym created to help standardise clinical handover. Aiming to reduce failures in communications, while improving the quality & safety of patient information from staff to staff member. This formula should be used to complete handover at Western Health.

In using ISBAR staff will support safe, coordinated care, by:

  • Standardising our approach to clinical handover
  • Identifying the patient
  • Explaining the patients situation and presentation
  • Explaining the patients medical and social background
  • Communicating any Alerts & Allergies
  • Reviewing results & requests
  • Detailing and communicating the current plan
  • Documenting and communicating current lines & devices
  • Including the handover sticker in the patients clinical notes
