Clinical Teams To Support MHAOD Care In SHED

This document provides a summary of best practice in relation to collaboration and communication between new clinical teams under SHED’s Mental Health, Alcohol and Other Drugs Model of Care (MHAOD MOC). The summary has been created to support staff during the Sunshine Hospital Emergency Department Redevelopment transition and early post-move period, and has been endorsed by the ED Leadership Team.

Overview

This QRG provides an overview of the Clinical Teams set out under SHED’s MHAOD MOC, and how these teams work collaboratively to support the ideal patient journey for patients presenting to ED with mental health, alcohol and other drugs (AOD), and/or social care needs.

Applicability

This QRG is applicable to all staff working in SHED.

Responsibility

The ED Leadership Team are responsible for communicating this QRG to staff.

Guideline/Process Details

SHED’s MHAOD MOC introduces front end streaming for patients with mental health, AOD and/or social care needs.

The MHAOD Patient Journey will be supported by the following clinical teams at SHED:

  • Emergency Department Triage and Arrival Team
  • Intake and Streaming Team
  • BAU Team
  • The Hub Team

These teams bring together clinical expertise from emergency medicine, emergency mental health, addiction medicine and care coordination – working collaboratively across the patient’s journey to achieve the best clinical and social outcomes.

Emergency Department Triage and Arrival Team

The Triage and Arrival Team work out of the Triage & Emergency Services arrival area. This team is unchanged by the implementation of the SHED MHAOD MOC.

All SHED patients are triaged using the Australasian Triage Scale (ATS). WH uses the Emergency Triage Education Kit (ETEK) to train SHED staff in the ATS, which includes a mental health triage learning unit.

Intake and Streaming Team

The Mental Health Nurse Navigator and an emergency medicine physician will comprise the Intake and Streaming Team, which supports the Triage and Arrival Team by being responsible for:

  • Risk screening, including timely initial mental health and behavioural screening of patients.
  • Patient streaming decisions.

Where required, this team will also play a role in therapeutic intervention for patients, including patients who are not directly admitted to a designated mental health point of care of the ED.

BAU Team

The BAU team includes clinicians from emergency medical and nursing, emergency mental health, addiction medicine and ACE. The team is responsible for providing care to patients who meet the eligibility admission criteria of BAU.

The Hub Team

The multidisciplinary Hub Team is nursing-led with medical support available from the main department.

Nursing staff, EMH, addiction medicine clinicians and ACE coordinators work collaboratively to provide therapeutic care to patients who meet the eligibility admission criteria of The Hub.
