The MHAOD Outreach Service will be accepting referrals from this Monday the 13th February
About the MHAOD Outreach Service
- The MHAOD outreach service is part of the Sunshine ED MHAOD Hub
- We are a multi-disciplinary team that engages with patients that have been discharged home from SHED that have mental health (MH) and or alcohol and other drug (AOD) care needs
- During the first phase we will provide support for up to 28 days after ED discharge via telehealth and move to face-to-face support in later stages
- The service intends to provide short term supports for patients with the aim of linking the patient into community based services
Who is this service for?
This service is for people who present to ED with MH and or AOD care needs that are being discharged home. The patient must consent to being referred to the service. They must also:
- Be aged between 18 and 64 years
- Reside within the Western Health catchment
- Mid West (Melton and Brimbank)
- South West (Wyndham, Hobsons Bay and Maribyrnong)
- Have no current suicidal ideation and do not pose any risk to themselves or others
And
Have undergone a comprehensive multidisciplinary assessment as appropriate under the SHED/MHAOD model of care
Or
- Have been assessed by EMH or ED medical as not requiring either medical or psychiatric inpatient admission or Area Mental Health Services community follow-up, including referral to the HOPE service
How to refer?
Referrals are made through BOSSnet
1.Make sure the patient meets eligibility criteria and consents to being referred
2.Log onto BOSSnet and complete a WH community services referral (left hand column)
3.When completing the referral make sure you state “MHAOD outreach” in the services required field
4.Submit referral and let the patient know they’ll receive a call within 48 business hours (Monday to Friday)
